While completing the survey, if you cannot advance to the next page, it means that you have missed one or more questions. Look for blinking asterisks for missing questions.
Please make sure to click the "Submit" button at the end of the survey, so that your responses are recorded in the system. Once you submit, you should be able to see a page like below:
